Permissions Form

I give permission for my:

Child’s name (forename only) and picture to be used in brochures, leaflets and displays published by La Moye School and the Department for Children, Young People, Education and Skills.*
Child’s name (forename only) and picture to be used by media when the school has asked them to record individual, class or whole school activities that are of interest to the general public, e.g the JEP, Parish Newsletter, local radio and television broadcasters.*
Child’s unnamed picture to appear on the school’s website in order to celebrate achievements or events.**
Child’s unnamed picture to appear on Facebook in order to celebrate achievements or events.**
Child’s name to be mentioned on the school website and Facebook for achievements etc, although not linked to any picture of them, in order to celebrate achievements or events.**
Child’s name to be given to School Photographer so that my child’s picture can be taken for individual and class photos and used on the school’s management information system, SIMS.*
Child’s name, date of birth and contact details to be given to Jersey Health and Social Services Department and Family Nursing and Home Care for dental, visual and health screening including school age immunization.*
Contact details to be used to receive information through In-touch and in school newsletters, about fundraising events such as ‘Cake Sale for BBC Children in Need’ or to receive flyers and information from other Data Controllers such as, but not limited to, Jersey Sport, Jersey Music Service and La Moye School Parent Teacher Association. (Under the new law this is considered Direct Marketing, which is why we need your express permission.)*
Contact details to be passed to the La Moye School PTA if my child is attending a PTA event and I need to be contacted in the event of an emergency or to collect them early, for example due to illness.*
I give permission for my child’s name, date of birth and assessment data to be processed by GL Assessment as part of La Moye School’s academic arrangements. I understand that this data is only accessible by staff at La Moye.**
I give permission for my child’s assessment data to be processed by BSquared Ltd as part of La Moye School’s academic monitoring arrangements. I understand that this data is only accessible by staff at La Moye School and used to target children’s learning and monitoring progress. It is not available to other schools or the wider community.**
I give permission for my child’s name to be processed by Google Suite and/or Seesaw as part of La Moye School’s academic arrangements to support and enhance learning (essential for home-schooling in the event of another wave of Covid-19).**
I give permission for my child’s name to be processed by Times Tables Rock Stars and NumBots as part of La Moye School’s academic arrangements to support and enhance learning.**
I give permission for my child’s assessment data to be processed by Early Excellence Ltd as part of La Moye School’s academic monitoring and reporting arrangements in Foundation Stage. I understand that this data is only accessible by staff at La Moye School and used to target children’s learning and monitoring progress. It is not available to other schools or the wider community.**
I give permission for my child’s name to be uploaded to Junior Librarian (MLC systems Ltd) for the purpose of managing the library and issuing books to children.**
